چکیده

Although a primary tenet of neurosurgical oncology is that survival can improve with greater tumor resection, this principle must be tempered by the potential for functional loss after radical removal. Current neurosurgical innovations aim to improve our anatomical, physiological, and functional understanding of the surgical region of interest to prevent potential neurological morbidity during resection. We review current and future imaging modalities as well as state-of-theart intraoperative techniques that can facilitate extent of resection while minimizing the associated morbidity profile.

Initial Experience with Brain Mapping under Awake Craniotomy for Resection of Insular Gliomas of the Dominant Hemisphere

Background & Importance: Insular lobe is located at the depth of sylvian fissure and is hidden by frontal, temporal and parietal lobes in close vicinity of internal capsule and basal ganglia and adjacent to the speech centers in the dominant hemisphere. Thus, radical resection of insular gliomas can be even more difficult. Brain mapping techniques can be used to maximize the extent of...

Aggressive angiomyxoma of the vulva: a case report

Background: Aggressive angiomyxoma is a rare mesenchymal tumor with extensive local invasion. It often presents with a lump in the perineal area and pelvic. Since it often does not involve adjacent organs such as the urethra and anus, does not cause obstructive symptoms. Its incidence is 6 times higher in women than men. This tumor grows slowly and has a slight tendency to metastasize. However,...

Assessment of Surgeon Judgment during Resection of Laryngeal Carcinoma

Carbon dioxide (CO2) laser surgery as a conservative tool plays a peculiar role in the management of head and neck cancer. Numerous patients who were candidates for transoral laryngeal microsurgery have forced us to eliminate frozen-section evaluation of surgical margins and use a magnified view of the larynx. The present study evaluated surgeon-judged negative margins with permanent microscopi...

Lessons from Brain Mapping in Surgery for Low-Grade Gliomas: Study of Cerebral Connectivity and Plasticity

Radical surgical removal of WHO grade II glioma, when possible, appears currently the first treatment to delay the risk of anaplastic transformation. The goal of surgery is to maximize the quality of resection, while minimizing the risk of permanent neurological deficit. Thus, due to the frequent location of these lesions in eloquent areas, and because of an important interindividual anatomofun...

An Effective Technique for Endoscopic Resection of Advanced Stage Angiofibroma

Introduction: In recent years, the surgical management of angiofibroma has been greatly influenced by the use of endoscopic techniques. However, large tumors that extend into difficult anatomic sites present major challenges for management by either endoscopy or an open-surgery approach which needs new technique for the complete en block resection.   Materials and Methods: In a prospective obse...
